Sequence for a routine exam
- PW at LVOT to derive stroke volume (with CSA_LVOT).
- CW across AV, MV, TV, PV for peak velocities and gradients.
- PW at mitral inflow (leaflet tips) for E, A, DT.
- Tissue Doppler at septal and lateral mitral annulus for e′ (diastolic function).
- PW of pulmonary vein (S, D, atrial reversal).
- PW of hepatic vein and IVC (RA pressure estimation).
- Color Doppler for regurgitant/shunt lesions in every view.
Age-related changes in transmitral inflow
- Young adults: E > A, E/A > 1.
- Middle age: E/A ≈ 1.
- Older adults: E/A < 1 (impaired relaxation is the norm).
- With age: E velocity ↓, deceleration time ↑, A velocity ↑.
